Medical records have a variety of uses by a wide variety of personnel. Within the guidelines of confidentiality and patient interest, the scope of the medical record as a resource material is limited only by the quality of the content and the needs of the prospective users.

RELEASE OF RECORDS

Request for a medical certificate or clinical information when the patient is still confined shall be referred to the attending physician.

Certificate of confinement, signed by the attending physician, shall be issued while the patient is still confined in the health care facility.

The medical certificate shall be signed by the attending physician when the patient is already discharged.

The medico-legal certificate shall be duly signed by the attending physician.

Certified photocopies of a patient’s health record shall be released, but limited to the following:

Discharge summary/ Medical abstract

Laboratory and diagnostic results

Report of operation

Discharge Summary/ Medical Abstract will be released subject to availability. To avoid delay, patients are encouraged to request before discharge.
Requests via Text/Call or email will not be entertained as we cannot verify the identity of the requesting party.
For an authorized representative, an authorization letter from the patient, along with a photocopy of any government-issued ID bearing the patient’s specimen signature, is required, as well as a valid ID of the authorized representative.
